OUR STORY

In 2004, my mother was diagnosed with dementia. In spite of round-the-clock, in-home care, she was visibly isolated and her mind wasn’t being appropriately stimulated. I began looking for a home that would offer specialized, personalized dementia care in a safe, stimulating and welcoming environment. Unfortunately, it simply didn’t exist. I was motivated and needed to found Syncare in Plymouth Minnesota. From the very beginning, the company’s vision – and promise – was to support, enrich, and empower the lives of our residents. And that’s exactly what we do.

ANN HORTILLOSA

Ann Hortillosa is the CEO and Owner of Syncare Memory Suites, bringing together decades of clinical expertise, healthcare leadership, innovation, and a deeply personal commitment to compassionate care.
 
Ann immigrated to the United States from the Philippines in 1989 to work as a Registered Nurse at the world-renowned St. Luke’s Hospital and Texas Heart Institute. That experience shaped the foundation of her career: excellence in clinical care, respect for every patient, and a deep understanding of the trust families place in those who care for their loved ones.
 
Over the years, Ann expanded her leadership in healthcare, serving as a hospital leader and later as CEO of a multi-state medical staffing company. These roles strengthened her ability to identify, recruit, and develop exceptional caregivers & professionals who bring clinical skill, reliability, integrity, and a genuine heart for service.
 
For nearly a decade, Ann has led Syncare with a hands-on approach rooted in accountability, excellence, and dignity. Today, she oversees three Syncare locations, ensuring that each home reflects high standards of safety, personalized care, warmth, and respect.
 
Drawing from her clinical background, leadership experience, and understanding of family needs, Ann helped shape Syncare’s Living Well Program, an innovative dementia care framework adapted at Syncare to support residents in a more personal, meaningful, and human-centered way. This new approach emphasizes dignity, safety, purposeful engagement, individualized support, emotional connection, and daily routines that help residents feel known, valued, and secure.
 
At Syncare Memory Suites, Ann has built a residential memory care environment where residents feel safe, familiar, and respected, and where families can feel confident their loved ones are cared for with patience and dignity. Her commitment to quality is reflected in every detail, from the caregivers she selects to the home-like spaces she thoughtfully creates.
 
Ann has also played an active role in the Harvard alumni and business leadership community. She formerly served as Co-Chair of the Harvard Business School Minnesota Chapter, co-founded the Harvard CEO Series in Minnesota, and served as a Board Member of the Harvard Club of Minnesota.
 
Outside of her professional work, Ann enjoys gourmet cooking, baking, traveling, and interior design—passions that reflect her appreciation for beauty, hospitality, and welcoming spaces.

ERIC PIRIUS

Eric was born in Red Wing, MN. He grew up in Alaska and attended college at Vassar University. He also worked and lived in Tokyo for 10 years. These unique experiences gave him creativity, resourcefulness and determination- traits that serve his clients well and his strong practice in finance. As Co-founder of Syncare, he is proud to provide the strategic backbone for the company’s growth and success. Eric is an avid biker who bikes even in the coldest days. He is a very competitive athlete.

To say we’re choosy is an understatement. We pride ourselves on hiring only high-caliber professionals who are certified in memory care. These are the kind of people we would happily invite over for dinner. Individually and collectively, they are compassionate, dedicated, and hard-working.

Registered Nurses

Each resident has a dedicated registered nurse (RN) who assesses and monitors your loved one’s health status on a weekly basis. Our RNs are accessible all hours of the day and night in the event our residents need them.

Resident Assistants

Our Resident Assistants (RA) are  highly trained, and Syncare’s ongoing training keeps them current on relevant practices for our residents.

Care Partners

We understand that the best care is collaborative. Nurses, physicians, resident assistants, occupational and physical therapists, and other care providers work as a team to offer our residents the very best care.
