Memory Care in Plymouth & Minnetonka, Minnesota
A small-home, nurse-led approach centered on helping each person continue to live well
When someone you love is living with Alzheimer’s disease, dementia, or another form of memory loss, finding the right care is about more than safety. Families want to know that their loved one will be known, respected, engaged, and supported as an individual.
Syncare Memory Suites & Assisted Living provides personalized residential memory care in Plymouth and Minnetonka, Minnesota. Our small-home approach combines 24-hour caregiver support, nursing and clinical oversight, meaningful daily engagement, movement, familiar routines, and close personal relationships in a comfortable residential setting. Rather than asking residents to fit into an institutional schedule, we take time to understand each person’s life story, preferences, abilities, natural rhythms, interests, and support needs. That understanding guides how we care for the whole person throughout the day.
Personalized Memory Care Built Around the Individual
No two people experience memory loss in exactly the same way. At Syncare, memory care begins with the person rather than a standardized schedule.
We learn about each resident’s family, past roles, interests, abilities, preferred routines, sleep and wake patterns, favorite foods, communication style, and the things that bring comfort or meaning. This information helps our team provide support that feels familiar and personal.
Whether we are assisting with dressing, meals, mobility, medications, personal care, or daily activities, our goal is to support independence whenever it is safe and appropriate while providing the level of help each resident needs. Our caregivers are encouraged to slow down, communicate respectfully, offer choices, and build genuine relationships. Learn more about our care team.
The Syncare Living Well Program
At Syncare, memory care is not simply about managing symptoms or completing a list of care tasks. Our Living Well Program asks a more human question: How can we help this person continue to experience a meaningful day?
Living Well at Syncare
Purpose + Daily Rhythm + Belonging + Meaningful Engagement + Movement + Comfort and Sensory Support
Purpose
People continue to need purpose throughout life. Residents are encouraged to participate in meaningful roles and everyday activities that fit their abilities and interests. This may include helping set a table, folding towels, watering plants, preparing food, organizing familiar items, gardening, or taking part in another activity connected to the person’s previous life and identity. The goal is not simply to keep someone busy, but to create opportunities to contribute, participate, and feel valued.
Daily Rhythm
People living with dementia do not all function best on the same schedule. We pay attention to when a resident naturally prefers to wake, eat, rest, move, socialize, participate in activities, or have quiet time. Familiar and predictable rhythms can provide reassurance, and individualized routines allow care to follow the person rather than the clock whenever possible.
Belonging and Relationships
Human connection remains important even as memory changes. A small residential environment gives caregivers more opportunity to learn how each resident communicates, what brings comfort, what may cause frustration, and how the person prefers to interact. Shared meals, conversation, music, family involvement, familiar routines, and everyday moments all create opportunities for connection and belonging. Families can also explore our family support resources.
Meaningful Engagement
Activities are individualized around the resident’s interests, history, abilities, mood, and energy level. A meaningful experience may include music, reminiscence, cooking, art, sensory activities, conversation, household tasks, puzzles, photographs, time outdoors, or quiet companionship. What matters is whether the activity is meaningful to the individual, not whether it simply fills a calendar.
Movement
Movement is woven into daily life based on each person’s abilities and care needs. Residents may be encouraged to walk, stretch, complete range-of-motion activities, participate in seated movement, or engage in other appropriate physical activity. The aim is to help residents continue using the abilities they have while supporting comfort, mobility, participation, and independence whenever possible.
Comfort and Sensory Support
Lighting, noise, fatigue, hunger, pain, unfamiliar surroundings, overstimulation, and changes in routine can all affect how a person living with dementia feels and responds. Syncare uses a calm residential environment together with individualized sensory and non-pharmacologic approaches when appropriate. Music, familiar objects, quiet spaces, comforting routines, movement, visual cues, and other strategies may be incorporated into the day. Understanding the Person Behind the Diagnosis
A diagnosis of dementia tells us something about a person’s medical condition. It does not tell us who that person is. Families help us understand the individual more completely so care can remain personal and connected to the resident’s identity.
- What did they enjoy doing and what roles were important to them?
- What music, foods, traditions, or routines are familiar?
- Are they naturally an early riser or a night owl?
- Do they enjoy conversation or prefer quieter surroundings?
- What tends to comfort them when they are anxious or unsettled?
- What abilities, interests, and relationships can we continue to support?
How Does Syncare Help Residents Feel Safe?
Safety is essential in memory care, but safety should not come at the expense of dignity. Our memory-care homes use appropriate safety measures while maintaining a comfortable residential atmosphere.
Caregivers are available around the clock to provide assistance and supervision based on each resident’s needs. Our team also looks beyond a behavior itself and considers what the resident may be communicating through changes in mood, restlessness, anxiety, or withdrawal.
A calm environment, familiar caregivers, predictable routines, appropriate movement, meaningful engagement, and respectful communication can all contribute to a greater sense of security.

Is It Time to Consider Memory Care?
Memory care may be worth exploring when several concerns are occurring repeatedly or when a loved one needs increasing supervision or support.
- Increasing confusion or getting lost in familiar places
- Medication mistakes or missed doses
- Falls or other recurring safety concerns
- Difficulty preparing meals or maintaining nutrition
- Changes in hygiene or dressing
- Increased anxiety, agitation, or withdrawal
- Reversal of sleep patterns or nighttime wakefulness
- Difficulty completing familiar tasks
- Caregiver exhaustion or inability to safely meet needs at home
One incident does not necessarily mean someone needs memory care. Patterns and changes over time are more informative, and medical concerns should be discussed with the appropriate health care professional.

Memory Care Should Still Feel Like Living
Choosing memory care does not mean that meaningful living ends. At Syncare, we believe the question should not only be “What care does this person need?” It should also be “What will help this person live well today?”
Even when memory changes, there can still be connection, purpose, laughter, movement, music, comfort, relationships, familiar routines, and moments that matter. That is what Living Well means at Syncare.
